Prohibits the resale or commercial use of the site or its content, as well as the collection and use of product listings or descriptions. It also prohibits using data mining, robots, or similar data-gathering methods and downloading any portion of the site except as expressly permitted.

 

      Waived Mobile Testing, L.L.C., is responsible for the drawing and delivery of laboratory specimens. WMT does not collect any specimens without a signed physician order. Waived Mobile Testing, L.L.C. is not allowed to give any medical advice, treat, or diagnose anyone.

 

Waived Mobile Testing, L.L.C., needs patient information, a copy of the order, and a copy of the insurance card (front and back) 48 hours before the scheduled appointment. If we do not receive the requested information within the time range mentioned, then the scheduled appointment will be considered canceled. This is extremely important so that we have adequate time to prepare for the blood draw and perform our job correctly.

 

Waived Mobile Testing, L.L.C. handles the delivery of all samples to their proper laboratory. If a sample is being collected for a laboratory that needs to be picked up via courier service, WMT is not responsible for any delay in arrival due to weather conditions or any other problems that may occur once the sample has been delivered or picked up by courier service.

 

Waived Mobile Testing, L.L.C. is not responsible for any errors that may occur at the laboratory. Once received by WMT, the lab will process the specimen. In the event of a lab error, WMT will gladly return to collect the sample at a discounted rate. Any error made by AMPS will be corrected by re-drawing at no extra charge, as long as all correct paperwork was submitted adequately prior to the appointment.

 

Any collection errors caused by Waived Mobile Testing, L.L.C. (e.g., drawing the wrong tubes, unable to obtain a satisfactory blood flow to collect the specimen, etc.) will not incur a charge to the patient or will be redrawn at no additional charge to the patient.

 

Waived Mobile Testing, L.L.C., makes every attempt to have contact with the patients prior to the draw. We do not show up unless we have made verbal contact with the patient on the morning of the scheduled blood draw.

 

Please be considerate. In the event an appointment needs to be canceled, Waived Mobile Testing, L.L.C., needs to be given a 24-hour notice. You may reach us by phone, email, or text.

 

If we arrive at the appointment for the blood draw and are unable to collect the specimen due to circumstances beyond our control – for example (the patient refuses, a fasting blood draw was scheduled, but the patient ate food, the patient did not drink plenty of water, the patient gets taken to the hospital after they have ordered STAT blood work or not available to be drawn, etc.) the patient will remain responsible for the full charges of the draw. 

 

Any billing questions or problems for tests collected for this service should be directed to your insurance company or the laboratory billing department. Payment for services provided by Waived Mobile Testing, L.L.C., is collected on the day prior to the appointment. Waived Mobile Testing, L.L.C. does not bill insurance companies, and any claims must be submitted by the client with no guarantee of reimbursement.

 

Waived Mobile Testing, L.L.C. is a private pay service. It provides specimen collection services and bills for laboratory testing through your insurance. Payments must be prepaid before the time of service.

 

Please be advised that Waived Mobile Testing, L.L.C., is not responsible for any outstanding balances with LabCorp or Quest. These balances must be settled prior to a scheduled home lab draw. The labs have the right to decline specimens of patients with outstanding balances.

 

Waived Mobile Testing, L.L.C. is an independent convenience service not affiliated with laboratories or doctor’s offices. It is not authorized to give discounts for lab tests being collected.

We are fully HIPPA compliant and respect the privacy of the clients we serve. Under no circumstances will Waived Mobile Testing, L.L.C., give out clients’ personal information to unauthorized personnel. All clients’ information is always kept personal and confidential.

 

Waived Mobile Testing, L.L.C., does not receive any results. If your doctor did not receive results, it is the responsibility of the Doctor’s office to get in touch with the lab. Most of the time, they have an account with the labs.

 

*For parents with children (Ages 12 and up):

If your child is getting their blood drawn, you must do your part. It is important to keep them hydrated 24 hours prior to the scheduled blood draw.

 

The less stress in the room, the higher the success rate. We are almost always successful on the first try; HOWEVER, it may take up to three tries at times. This needs to be understood. This is normal, especially with tiny veins and a moving target. We understand how stressful it can be for you and your child during their blood draw. Please understand how stressful it can be for us if we do not have understanding parents.

 

Safety is very important. If you know that your child gets aggressive when upset, is difficult to draw, or normally takes more than one person to hold them still, please let us know so that we can plan to have a safe and successful draw.
